3歳児から手足延長手術が可能です。一般的に、身長が低いなどの発達上の問題による手術は若いうちに行われます。さまざまな理由で片脚が短い場合、状況によってはできるだけ早く治療を開始する方が合理的です。ですが、発育段階がまだ完了していないため、美容上の脚延長の場合、当組織では18～19歳までを待つことをご提案しています。
精神的・肉体的な準備をしておくと、手術の全行程で役に立ちます。最初にすべきことは、手術についてできるだけ多くの情報を得ることです。次に、専門家に相談して、自分が手術を受けることができるかどうかを確認することです。次に、筋肉をストレッチしたり、リラックスさせたりして、体のエクササイズを行います。
脚延長手術では、個人の骨の厚み、骨格、健康状態などを考慮し、最もリスクの少ない長さと方法を選択する必要があります。手術法を選択する際には、すべての手術法とそのメリット・デメリット、どの方法が自分に合っているかなどを主治医と相談するべきです。
医学的な理由による手術では、医師が長さの割合を決定します。大きなリスクがなければ、通常は、骨の長さの5分の1まで伸ばすことができます。例えば、太ももの骨の長さが50センチならば、10センチほど伸ばせるということです。ですが、長さが20センチの子供の太ももの骨を10センチも伸ばすのは危険すぎます。そこで、このような子どもの場合は、5分の1の基準に従い4センチほど伸ばします。 必要以上に伸ばしてしまうと、筋肉の硬直、神経の損傷、血管への負担、骨の感染など、さまざまな問題が発生する恐れがあります。つまり、身長をどれくらい伸ばすかは、本人の期待や夢よりも、骨の長さ、構造、質、医師の勧めによって決まります。このため、美容的脚延長手術において、本人の希望は重要ではありますが、患者がリスクを負わないような長さを医師が提案する必要があります。成人の場合、この長さは、太ももの骨（大腿骨）で約8センチ、脛骨で約6センチです。こうした長さには個人差がありますが、治療過程で発生し得る合併症に応じて変わる場合があります。
創外固定器を使用する方法では傷跡が多くなりますが、体内で装着する方法ではそれほど多くありません。
1週間ほど入院するのが一般的です。3～4週間、脚にギプスを装着することもあります。手術自体は、準備時間、麻酔、麻酔後の回復までの時間を含めて5時間程度かかります。また、完治するまで6ヶ月かかります。この時点で、日常の生活に戻ることができます。また、十分に時間をかけずに手術を行ったり、過度に伸ばす手術を行ったりした場合は、治癒の過程に悪影響を及ぼします。一方、治癒に最もよい影響を与えるのは、手術後に理学療法の運動を定期的に行うことです。
脚延長手術は、手術や治療の過程で少し痛みを伴います。特に、術後初期には強い痛みを感じることがあります。その際には病院で薬を静脈に投与しますし、その後は医師が勧める鎮痛剤で痛みを緩和することができます。治療を続ける中で痛みを軽減するには、定期的な理学療法と医師から勧められた運動を行う以外にありません。運動で骨と筋肉の組織を強化すれば、痛みの軽減に役立ちます。反対に、理学療法のプログラムをきちんと行わないと、長期間固定されていた関節を動かそうとする際に痛みを伴います。
座っていることの多い仕事の場合は、退院後すぐに仕事に復帰できますが、忙しく動き回る仕事の場合は、平均3〜6ヶ月の休職が必要です。
スポーツによります。ただし、重量挙げのような激しいスポーツは最低1年待った方がよいでしょう。軽いスポーツは、医師の指導のもと、6ヵ月後から可能です。
選択する手術法によっては、器具を取り外すための再手術が必要になることもあります。例えば、体内の髄内釘は、患者さんの希望で一生骨の中に残すこともあれば、手術後1～2年で取り外すこともあります。 これとは別に、治療過程で発生し得る合併症のせいで、追加の外科手術が必要になることもあります。例えば、延長部分の癒合が十分でない場合、手術室で幹細胞などを利用して、癒合を促進させることが可能です。 ここで重要なのは、こうしたプロセスをすべて事前に話し合い、問題が発生した場合に適切な方法、そして適切なタイミングで対処できるようにすることです。
これから脚延長手術を受ける人は、以下に挙げる問題を知っておくとよいでしょう。
感染症
  • 延長部分の癒合の遅れまたは不全
  • 静脈での血栓形成
  • 関節のこわばり
  • 精神的問題
  • 創傷部位の傷跡
  • 激しい痛み
    • 治療の中断にもつながる最大の問題は二つあります、一つは、感染症です。特に複合法の場合には、手術による感染、または手術後に感染が起こったら、直ちに治療を中止するか、変更する必要があります。もう一つは、延長処置中の循環器系や神経に関連する問題です。例えば、いつまでたっても運動中に脚の感覚を十分に感じられない場合、これは重要なサインです。このような場合は、治療を継続せず、延長処置を中断する必要があります。 これとは別に、患者さんに関係する問題もあります。推奨される運動プログラムを行って、関節の動きを維持する必要があります。関節の動きに問題がある場合は、リハビリテーションを強化しますが、効果が得られない場合は治療を中止します。また、伸ばした長さによっては、膝裏の筋肉や足首の後ろのアキレス腱が短くなり、歩行時の膝の曲げ伸ばしや、つま先立ちなどに問題が生じることがあります。 このような場合、理学療法で問題を克服できないようであれば、目標とする長さへの到達が困難になることもあります。
    手術後、自宅でも定期的かつ適切に理学療法を行うことが重要です。4～6週間にわたり、週に何日かは自宅での理学療法が必要となるでしょう。
    新しい骨が完全に癒合すれば、延長手術前の普段の生活に戻ることができます。ただし、最初は、新しい筋肉の長さに慣れる必要がありますが、それほど時間はかからないと思います。
    先天性の変形は遺伝性の疾患でも見られますが、後天的な疾患や外傷が原因で変形が生じることもあります。
    変形の矯正手術に適切でない年齢というのはありません。その時点で、成長とともに変形が改善するかどうか、悪化するかどうかに基づき計画を立て、治療の提案をする必要があります。 治療計画には骨年齢が非常に重要ですが、こうした治療は、変形の程度によって何歳ででも計画することができます。
    これらの手術の後には、理学療法とリハビリテーションが絶対に必要不可欠です。手術の内容によっては、筋力の低下や関節の動きの制限を経験することがあります。 この過程を早く克服するためには、早期の理学療法が非常に有益です。
